The Care Progression Manager serves as an integral component of the Capacity Leadership Team for the Health System while owning primary oversight for case management at Saint Luke’s East Hospital. This position reports directly to the Vice President of Patient Progression.
This position is responsible for the management direction and coordination of Case Management and Social Work components that comprise the Care Progression Department. The Manager plans, directs and implements all services related to case coordination, concurrent review, and utilization management programs, ensuring compliance with TJC, state and other regulatory agencies.
Responsibilities include but are not limited to:
Managing designated aspects of the case coordination, prior authorization, clinical documentation, and concurrent review programs to ensure maximal reimbursement, prevent financial loss to the hospital, and to minimize patient liability
Serving as an escalation point for complex discharges
Managing individual(s) including but not limited to: hires, trains, assigns work, manages & evaluates performance, conducts professional development plans.
Ensuring that the productivity and actions of that group meet/support the overall operational goals of the department as established by department leadership
Developing and adhering to departmental staffing, revenue and/or expense budgets
Responding to changes in the business which may affect the ability to achieve the budgets goals
Adjusting programs to support the needs of the target patient population
