LVN - Wound Clinic - Per Diem

Shift: 8:00am - 4:30pm

2+ years of experience as an LVN is Required

PATIENT POPULATION:
The patient population served includes geriatric adolescents and adults. This also includes services which affect facility staff, physicians, visitors, vendors and the general public.

Under the direction of the department Leader(s), the Wound Care, Licensed Vocational Nurse, (L.V.N.) is responsible for all assigned nursing care activities to include but not limited to wound care for patients of all ages and other clinical activities associated with providing safe patient care. The L.V.N. is responsible for following all WHC policies and procedures, care protocols and Diversified Clinical Services Practice Guidelines, as directed by a Registered Nurse, or Physician. The L.V.N. will be responsible for assisting with patient and wound care data conferences, and continuing education is required as determined by the Clinical Nurse Manager.

Must be able to work normal/scheduled working hours to include Holidays, call-backs, weeknights, weekends, and on-call. Agrees to participate, as directed, in emergencies and community disasters during scheduled and unscheduled hours. As a designated disaster service worker you are required to assist in times of need pursuant to the California Emergency Services Act.

EDUCATION/TRAINING/EXPERIENCE:
To perform this job successfully, an individual must have a nursing degree from an accredited nursing school. Must have and maintain a current license in the state of California. Minimum two (2) years of experience as LVN required Wound care experience preferred. Familiarity with PC's and computerized nursing systems and tools, or demonstrated ability to learn. Basic knowledge of regulatory/accrediting requirements for healthcare organizations.

Must have strong interpersonal skills to interact positively and effectively with patients, visitors, staff, and physicians, good time management skills, and demonstrated ability in assessment and critical thinking.

Ability to complete assignments in a timely manner, accuracy in reporting and documenting information, and professional accountability.

Ability to interact with and maintain a respectful, positive and friendly manner with supervisors, co-workers and internal and external customers to include being flexible with work schedules and requests; keeping commitments; keeping others informed of work progress, and being open to and sharing ideas as needed to handle complaint situations, explain policies and procedures; addressing others by name, title or other respectful identifier, and respecting the diversity of our workforce in actions, words and deeds.

LICENSURE/CERTIFICATIONS:
Must have and maintain current license with the state of California. Must maintain BLS credentialing and all unit competencies. Must complete Diversified Basic Wound Care Course within six (6) month of employment.

Responsibilities and Essential Functions:

1 Must be able to demonstrate the knowledge and skills necessary to provide care to patients served on his/her assigned unit as described in unit based competencies, policies and procedures.
2 In collaboration with other staff members, provides bedside patient care, implementing the interventions identified in the plan of care, as directed by R.N.
3 Ensures appropriate patient care documentation. Accuracy in reporting and documenting information.
4 Implements and assures compliance with wound center and hospital policies and procedures. Maintains familiarity with regulatory standards.
5 Communicates with staff and physicians to ensure quality of care. Reports quality of care issues to staff R.N. or Program Director.
6 Ensures availability of necessary equipment and supplies.
7 Participates in own employee feedback session identifying individual strengths and areas of growth.
8 Identifies limitations in own skill/knowledge and seeks consultation from available resources to improve them.
9 Demonstrates commitment to ongoing professional growth and education. Professional accountability.
10 Must be able to operate all equipment that is essential to performing job responsibilities. Must have manual dexterity and ability to perform difficult and fine hand manipulation skills.
11 Must be able to walk and stand continually throughout shift. Must be able to frequently life, pull, and push patients with assistance.
12 Must have functional senses, including sight, speech, and hearing for care delivery and effective communication for quality of care.
13 Must have ability to wear protective clothing as needed.
14 Able to twist, turn and bend repetitively throughout the day. Able to crouch, stoop and reach high places. Able to carry, push, pull and handle patients in addition to multiple pieces of equipment.
15 Able to visually accommodate with good color vision, depth perception and wide field of vision.
16 Ability to complete assignments in a timely manner. Accuracy in reporting and documenting information. Professional accountability.
17 Able to talk and communicate well with all professionals.
18 Be in attendance on site and arrive to work on time
