LVN or LPN (Home Health)

Shifts Available: PRN & Full Time

Service Areas for Coverage:

• Northwest (Spring/Cypress, Tomball, Willowbrook, Inwood, Heights, Jersey Village)
• Northeast (Crosby, Atascocita, Kingwood, Humble)
• Southwest (59-288)

Responsibilities:

• Collaborate with the home healthcare team to develop and implement individualized care plans.
• Complete home care nursing visits.
• Prepare timely, complete, and accurate clinical notes.
• Communicate with healthcare providers and other care team members.
• Educate patients and their families about health conditions, treatment plans, and self-care techniques.
• Adhere to all agency policies, procedures, and regulatory requirements related to home health care.

Qualifications:

• Valid and current LVN or LPN license in the state of Texas or compact state.
• Minimum of one (1) year of experience in a home health setting.
• Bilingual (Fluent in Spanish) encouraged to apply
• Knowledge of home health regulations and general healthcare compliance.
• Experience with HCHB EMR. (preferred but not required)
• Excellent communication and people skills to effectively interact with patients, families, and healthcare team members.
