Moraga Post Acute is seeking a compassionate, dependable, and dedicated Licensed Vocational Nurse (LVN) to join our team on a Full-Time PM shift.

Position Details

  • Position: Licensed Vocational Nurse (LVN)
  • Employment Type: Full-Time
  • Shift: PM Shift
  • Location: Moraga Post Acute
  • Pay Rate: $38.00 per hour

Responsibilities

  • Provide compassionate, high-quality nursing care to residents in accordance with their individualized care plans.
  • Administer medications and treatments as ordered by physicians and in accordance with facility policies.
  • Monitor residents' conditions and promptly report changes to the appropriate healthcare professionals.
  • Perform nursing assessments and document resident care accurately and timely.
  • Collaborate with CNAs, RNs, physicians, therapists, and other members of the healthcare team.
  • Assist with developing and implementing resident care plans.
  • Provide wound care and other treatments as directed.
  • Monitor vital signs and other clinical indicators as needed.
  • Maintain accurate and complete nursing documentation.
  • Communicate effectively with residents, families, and healthcare providers.
  • Promote resident safety, dignity, privacy, and independence.
  • Follow all facility policies, procedures, and applicable healthcare regulations.
  • Respond appropriately to emergencies and changes in resident condition.
  • Provide guidance and support to nursing assistants and other care team members as appropriate.
  • Perform other nursing-related duties as assigned.

Qualifications

  • Current and valid California LVN license.
  • Previous skilled nursing or long-term care experience preferred.
  • Strong clinical, organizational, and communication skills.
  • Knowledge of medication administration, treatments, and nursing procedures.
  • Ability to recognize and respond appropriately to changes in resident condition.
  • Compassionate and respectful approach to resident care.
  • Ability to work effectively as part of a multidisciplinary team.
  • Dependable, professional, and able to work independently.
  • Strong attention to detail and accurate documentation skills.
  • Commitment to providing safe, quality, and person-centered care.
