Job Description

The LPN/RN position will work remotely to contact patients and provide non-face to face support to patients who are registered with remote patient monitoring (RPM). This is a hybrid position requiring regular on-site work at our Mid Valley Practice in Jermyn, PA. The ideal candidate would be local to northeastern Pennsylvania.

REPORTING RELATIONSHIPS

This position reports to the Director of Geriatric Service Line.

ESSENTIAL JOB DUTIES AND FUNCTIONS

While living and demonstrating our Core Values, the LPN/RN RPM & CCM will:

  • Ensure the consent and education was completed by the Community Health Worker
  • Monitor patients physiologic parameters
  • Manage assigned patient vital sign monitoring and documentation
  • Ensure transmission of vital signs from remote devices to Humhealth
  • Manage documentation in Medent and HumHealth
  • Ensure vital signs are captured for over 16 days or 20+ minutes
  • Initiate communication with patients to ensure patient minutes are completed for the month
  • Ensure patient needs are met by providing quality chronic care management
  • Ensure chronic care management needs are met for each assigned patient and communicated to the clinician as appropriate
  • Ensure patient needs are met to provide quality care coordination
  • Complete medication reconciliation
  • Update the clinician regarding patient compliance or noncompliance with medications
  • Send medication refills as patient requests
  • Coordinate care with the clinician
  • Ensure that all information that applies to the patient is documented in the EMR
  • Ensure patient care plan is accurate and up to date
  • Practice HIPAA confidentiality and security measures at all times during office hours and outside the office
  • Demonstrate responsibility for self-learning through participation in continuing education activities and conferences
  • Serve as clinical resource for staff, clients and families
  • Perform all other duties as assigned

Requirements

REQUIREMENTS

  • Licensed Practical Nurse or Registered Nurse with 3-5 years’ experience in healthcare environment
  • Previous experience in healthcare environment required (minimum 3-years)
  • CPR Certification required
  • Proficiency in Microsoft Word, Excel, and PowerPoint
