The LPN/LVN Care Coordinator (LCC) will be responsible for calling in verbal orders on behalf of nursing team members, tracking receipt of signed orders, coordinating daily operations and scheduling needs, and ensuring there is continuity of care. This role will establish and cultivate a positive working relationship with current referrals sources, field clinician, client families, and nursing team members to ensure workflow processes are achieved.
