Job Description

The Wellness Nurse (LPN) will report to Health & Wellness Director.

Schedule: Varies

Salary Range: $34.00 - $35.00 Hourly

Job Overview

Provide direct nursing care to the Residents, and to supervise the day-to-day nursing activities performed by care staff.

Responsibilities and Duties

• Supervise care staff in accordance with current state regulations and community policies
• Monitors the health, safety, and well-being of all residents
• Assisting in training and monitoring of medication administration
• Maintaining clinical quality assurance in accordance with federal, state and local standards
• Promote the highest degree of service to our residents while leading and demonstrating the mission of the company
• Participates in and attends all in-service training as scheduled
• Ensures compliance with all Federal, State, and local regulations, as well as community policies and procedures
• Other duties as assigned

Qualifications

• Qualifications
• Graduation form an accredited School of Nursing, with current license
• At least 2 years’ experience with long-term care, assisted living, home health or hospital setting preferred
• Experience working with residents with Alzheimer’s or other related dementias
• Ability to handle multiple tasks
• Knowledge of federal and state regulations, and of nursing practices, techniques and methods applied to health and wellness resident
• Proficiency in computer skills, Microsoft Office (Windows, Outlook, Excel) with the ability to learn new applications
• While performing the duties of this job, the associate is often required to stand, walk, sit, use fine and gross motor skills, reach with hands and arms, balance, stoop, kneel, crouch, talk, hear, and smell.
• An individual in this position will be required to lift or carry weight in up to 50 lbs. Intermittent physical activity includes lifting and supporting residents. The associate must use proper body mechanics.

The following Q&A provides clarification regarding eligibility, scheduling, compensation, and expectations for participation in the Weekend Warrior Program:

1. Are Team Members (TMs) in the community eligible to enroll in the Weekend Warrior Program?

Yes. Team Members working in the community are eligible to transition into the Weekend Warrior Program if they wish to give up the weekday work schedule/commitment. This is a weekend only position.

1. What days are considered part of the Weekend Warrior schedule?

Weekend Warrior schedules are individualized based on the needs of each community. The designated schedule may include a combination of Thursday through Monday, depending on community staffing needs.

The program is not limited exclusively to Saturday and Sunday. For example, a Team Member who consistently works a Thursday–Monday schedule may qualify as a Weekend Warrior. This is based upon community needs. ED determines the need.

1. What are the rates for the Weekend Warrior Program for each role?

Weekend Warrior rates are individualized based on the community's established hourly rates for each role. The Weekend Warrior rate should be set at $1.00 more per hour than the applicable standard hourly rate.

1. If a Team Member currently works only weekends, are they considered a Weekend Warrior and eligible for the increased rate?

Team Members who consistently work the designated Weekend Warrior schedule may qualify for the program and associated rate increase. Communities should first identify Team Members who are regularly scheduled to work every weekend and determine whether their existing schedule meets the Weekend Warrior criteria.

1. What are the scheduling guidelines for the Weekend Warrior Program?

The Weekend Warrior Program is intended for Team Members who are hired and scheduled to work weekends, rather than Team Members who simply pick up additional weekend shifts or work EOW, as part of their regular schedule. Working EOW along with their weekday schedule does not qualify for this program. However, a Team Member who is hired to work Thursday through Monday every week could meet the intended scheduling structure for the program. Again, ED determines the community's need.

The program is therefore based on a consistent, recurring weekend-focused schedule, not by occasionally picking up one or more additional weekend shifts. Those that are hired to work weekends only, either every weekend or some commitment of routinely scheduled weekends.
