Job Description

Job Title: LPN Visit Nurse | Take Your Nursing Into the Community | Home Health Company: Home Health Care, Inc. Location: Twin Cities Metro Area | Patient Visits Within Approximately a 35-Mile Radius of Your Home Schedule: Full-Time Compensation: $30–$38 per visit Benefits: Comprehensive Benefits Package + Mileage & Cell Phone Reimbursement To Stand Out Email Elena Ehrlich at elena.ehrlich@hhcare.net with your resume and a brief overview of your LPN experience, clinical background , and what you're hoping to find in your next nursing position. Your Nursing Career Doesn't Have to Stay Inside the Same Four Walls. Same nurses' station. Same hallways. Same call lights. Same feeling that five patients need you at exactly the same time. There is another way to use your LPN license. As an LPN Visit Nurse with Home Health Care, Inc. , your workday takes you into the community, providing skilled nursing care to patients where they actually live. You drive to your patient. Walk through the door. Focus on one person at a time. Provide the care they need. Then head to your next visit. Your office just happens to have a windshield. A Different Kind of Nursing Workday Home health gives you something that's difficult to find in a traditional facility: Space to focus. Instead of trying to divide your attention between an entire hallway of patients, you'll provide scheduled skilled nursing visits throughout your local territory. You'll assess. Treat. Educate. Listen. Document. And then move on to your next patient. There's independence—but you're never on your own. You'll have experienced RN Case Managers, Clinical Managers, physicians, therapists, and interdisciplinary clinicians working alongside you to coordinate each patient's care. And Your Territory Starts With Where You Live We don't want "Twin Cities Metro" to sound like you're expected to drive from one end of the metro to the other every day. Your patient visits will be scheduled within approximately a 35-mile radius of your home , allowing you to serve patients in communities closer to where you already live. That means your home location helps shape the territory you serve. Why LPNs Choose Home Health Provide meaningful 1:1 patient care Serve patients within approximately a 35-mile radius of your home Spend your day in the community instead of one building Practice with greater independence and autonomy Build real relationships with patients and caregivers Use your clinical skills in a variety of home environments Work independently while having an experienced clinical team behind you Make a direct impact on a patient's ability to remain safely at home What Will You Actually Do? Your visits may include: Skilled nursing assessments and observations Wound care and dressing changes Medication management and education Injections and other ordered nursing treatments Monitoring chronic conditions and changes in health status Reinforcing physician orders and treatment plans Teaching patients and caregivers about medications, diagnoses, and disease management Identifying concerns and communicating changes to the RN Case Manager or physician Collaborating with therapists and other interdisciplinary team members Maintaining accurate and timely clinical documentation Most importantly, you'll help patients continue receiving the skilled nursing care they need without leaving home. A Few Perks We're Especially Excited to Offer $30–$38 per visit Employer contributions toward medical insurance Employer-paid dental, vision, life insurance, and disability coverage Mileage reimbursement between patient visits Cell phone reimbursement for eligible clinicians Free access to our MedBridge Education Hub for continuing education and professional development Licensure reimbursement when it's time to renew Annual scrub reimbursement Three weeks of PTO plus six paid holidays Structured orientation, mentorship, and ongoing clinical support Opportunities for career growth throughout our family of healthcare companies You Don't Have to Come From Home Health Already have home health experience? Great. But if you've built your LPN career somewhere else, we'd still like to hear from you. Experience from environments such as: Skilled nursing TCU Rehabilitation Assisted living Hospice Clinic Other direct patient-care settings can provide a strong clinical foundation for home health nursing. We'll help you learn our processes and systems while you bring the nursing experience you've already developed. What We're Looking For Active Minnesota Licensed Practical Nurse (LPN) license Strong clinical and patient-care skills Confidence working independently in patients' homes Excellent communication and organization Compassionate, dependable, patient-centered approach Reliable vehicle, valid driver's license, and current auto insurance Ability to travel to patients within approximately a 35-mile radius of your home Home health experience preferred, but not required Why Home Health Care, Inc.? Since 1994 , Home Health Care, Inc. has helped Minnesota patients recover, manage chronic conditions, and maintain their independence at home. We're locally owned, Medicare-certified, and backed by more than 30 years of home health experience . We know field nursing requires independence. But we also know independence shouldn't mean feeling unsupported. That's why our LPNs work alongside experienced clinical leaders and interdisciplinary teams who are available when questions or patient needs arise. Maybe You Don't Need a Different Career. Maybe You Just Need a Different Setting. You can still be an LPN. Still provide skilled nursing care. Still use the clinical experience you've worked hard to build. But instead of spending your entire shift inside the same building, take your nursing into the community. One patient. One home. One visit at a time. $30–$38 per visit. Full-Time. Patient visits within approximately 35 miles of your home. Email Elena Ehrlich at elena.ehrlich@hhcare.net with your resume and a brief overview of your LPN experience. Your license stays the same. Your workday doesn't have to.
