Job Description

SIGN-ON BONUS ELIGIBLE!

Join our award-winning, 5-star hospital-based Skilled Nursing Unit. As an LPN in this high-acuity, post-acute setting, you will be the vital bridge between our Registered Nurses (RNs) and Nursing Assistants (CNAs). You will provide expert clinical care to patients recovering from surgeries, strokes, or acute illnesses, ensuring they receive the high-standard medical attention necessary for a successful transition home.

Key Responsibilities

  • Direct Patient Care: Provide bedside nursing care, including checking vital signs, monitoring intake/output, and assisting with activities of daily living (ADLs) as needed.
  • Medication & Treatments: Administer oral, topical, and injectable medications; perform wound care, dressing changes, and catheter insertions.
  • Clinical Monitoring: Observe and document changes in patient condition, reporting significant findings immediately to the supervising RN or physician.
  • Care Coordination: Assist in the implementation and revision of individualized care plans; ensure all treatments align with physician orders.
  • Patient Advocacy & Education: Teach patients and families about disease management, medication requirements, and post-discharge care.
  • Supervision: Oversee and delegate tasks to Certified Nursing Assistants (CNAs) to ensure comprehensive unit care.
  • Documentation: Maintain accurate, real-time records of vitals, treatments, and patient progress.

Shift: Full Time, 36 hrs. per week, 12 hr. shifts

Requirements

Qualifications

  • Education: Graduate of an accredited school of practical nursing.
  • Licensure: Current, valid LPN license in the state of NC.
  • Certification: Current Basic Life Support (BLS/CPR) certification required.
  • Experience: Previous experience in skilled nursing, long-term care, or rehabilitation preferred; new graduates with strong clinical rotations are encouraged to apply.
  • Skills: Strong critical thinking, emotional resilience, and the ability to communicate empathetically with stressed or vulnerable patients and families.

Physical Requirements

  • Must possess full range of body motion to pass basic FIT test for position to include walking, kneeling, standing, pushing, pulling, bending, stooping, reaching and sitting for extended periods of time
  • Must be able to lift, push, and pull a minimum of [30] pounds frequently.

When Iredell Memorial Hospital opened in 1954, we were a 100-bed hospital with a mission focused on the well-being of the community we serve.
