At LCMC Health, we are committed to providing compassionate, patient-centered care. We are seeking a dedicated Licensed Practical Nurse (LPN) Patient Navigator to serve as a trusted advocate, educator, and clinical resource for patients and families throughout their healthcare journey.
In this role, you will work collaboratively with physicians, care teams, and community partners to coordinate care, remove barriers to treatment, and ensure seamless patient experiences across the continuum of care. The LPN Patient Navigator functions as an integral member of a multidisciplinary healthcare team, serving as an advocate, interpreter, educator, counselor, and clinical liaison for patients. This role coordinates patient care from diagnosis through treatment and survivorship, facilitating communication among patients, families, providers, and support services.
The Patient Navigator provides expert nursing guidance, patient education, care coordination, and resource management while ensuring continuity of care and supporting optimal patient outcomes.
🎓 Graduate of an accredited Practical Nursing program.
📜 Current Licensed Practical Nurse (LPN) license through the Louisiana State Board of Practical Nurse Examiners
❤️ Current Basic Life Support (BLS) for Healthcare Providers certification through the American Heart Association
