Care Guide Partners PACE is a part of Hosparus Health. As a Program of All-Inclusive Care for the Elderly (PACE) provider, we work to empower people 55 and older with the support they need to stay in the homes they know and love as they age. PACE helps seniors who need nursing home-level care maintain their independence by providing all the medical care and support services they need to continue living comfortably and safely at home.

Schedule: 8:30am-5:00pm

The LPN Participant Navigator is responsible for supporting prospective participants, newly enrolled participants, and their caregivers throughout the PACE enrollment and transition process. This role serves as a clinical liaison between participants, caregivers, the Enrollment Team, and the Interdisciplinary Team (IDT) to ensure a seamless transition into the PACE program. The Participant Navigator helps identify and address barriers to enrollment, supports care coordination activities, promotes participant engagement, and provides ongoing navigation and follow-up during the first 90 days of enrollment to support successful integration into the PACE model of care.

Position Qualifications:
• Currently licensed as a Licensed Practical Nurse (LPN) through the Kentucky Board of Nursing or participating Compact State Board of Nursing.
• Minimum of two (2) years of nursing experience, preferably working with frail elderly populations, chronic disease management, geriatrics, home health, hospice, PACE, or care coordination.
• Working knowledge of Medicare, Medicaid, and healthcare eligibility requirements preferred.
• Experience coordinating care across multiple healthcare settings preferred.
• Strong communication and interpersonal skills with the ability to effectively interact with participants, caregivers, providers, and community partners.
• Strong organizational skills with the ability to manage multiple priorities and deadlines.
• Demonstrated problem-solving, critical thinking, and decision-making abilities.
• Ability to travel throughout the PACE service area.
• Must be a licensed driver.
