Baystate Health, a nationally recognized leader in healthcare quality and safety, and home to Baystate Medical Practices, the largest multispecialty group in the region is looking for a full-time Outpatient Behavioral Health LPN to join our experienced Behavioral Health team in Springfield, MA.
RESPONSIBILITIES:
Baystate Health Behavioral Health LPN's provide patient-centered clinical care under the supervision of a Nursing Supervisor, Physician, or Advanced Practice Provider in an outpatient setting. Responsibilities include preparing patients and exam rooms, performing patient intake and documentation in the electronic medical record, obtaining vital signs, administering immunizations and non-IV medications per protocol, and performing approved clinical procedures and point-of-care testing. The LPN assists providers during examinations and procedures, supports care coordination through patient education, follow-up, referrals, and remote patient monitoring, and collaborates closely with RNs and the interdisciplinary team to manage health maintenance, results, and registries.
