Job Description

Help Patients Transition Safely Home!

ChristianaCare HomeHealth is seeking a compassionate and relationship-driven Home Care Nurse Liaison (LPN) to serve as the key connection between referral sources, patients, families, and our HomeHealth team. In this role, you'll leverage your clinical expertise and communication skills to coordinate seamless transitions from hospital to home while ensuring patients receive the care and support they need. This role is Monday through Friday 8am-4:30pm with some 4 hour weekend days and holidays, depending upon the needs of the department.

What You'll Do

  • Collaborate with referral sources, discharge planners, case managers, and providers to coordinate HomeHealth services.
  • Review patient information and obtain required demographics, insurance, and clinical documentation.
  • Meet with patients and families to assess home care needs and support safe discharge planning.
  • Obtain and maintain consents, authorizations, Advance Directive information, and physician orders.
  • Coordinate with intake, scheduling, and clinical teams to ensure timely start of care.
  • Educate referral sources on ChristianaCare HomeHealth services.
  • Serve as a trusted resource for patients, families, and referral partners throughout the care transition process.

Qualifications

  • Current LPN license in the State of Delaware.
  • Knowledge of HomeHealth services, practices, and care transition processes.
  • Excellent communication, customer service, and relationship-building skills.
  • Strong organizational, problem-solving, and computer skills.
  • Ability to work independently in a fast-paced environment.
  • Valid driver's license and active auto insurance required.

Hourly Pay Range: $27.86 - $41.78

This pay rate/range represents ChristianaCare’s good faith and reasonable estimate of compensation at the time of posting. The actual salary within this range offered to a successful candidate will depend on individual factors including without limitation skills, relevant experience, and qualifications as they relate to specific job requirements.
