Job Description

We have a wonderful opportunity for full time and part time LPN's (day and evening shift) for the opening of our new Assisted Living Memory Care Unit.

The Nurse – Clinical & Unit Oversight is responsible for providing and coordinating nursing services while overseeing the day-to-day operation of the unit.

In addition to clinical responsibilities, the nurse provides daily leadership and oversight of resident care, the Meaningful Moments activity program, housekeeping/environmental services, and nutrition/dining services within the unit.

The nurse serves as the primary on-unit leader and works collaboratively with nursing staff, activity staff, housekeeping, nutrition services, families, and facility leadership to ensure residents receive safe, compassionate, person-centered care.

Essential Nursing Responsibilities

· Provide nursing services in accordance with Ohio Assisted Living/Residential Care Facility requirements, professional standards of practice, facility policies, and resident service plans.

· Complete nursing assessments and reassessments as required and monitor residents for changes in physical, cognitive, emotional, behavioral, and functional status.

· Notify physicians, advanced practice providers, families, and other appropriate individuals of significant changes as required.

· Ensure physician orders are implemented and followed; monitor medication administration and medication management practices.

· Provide treatments and nursing services within the nurse’s scope of practice.

· Monitor residents following falls, incidents, changes in condition, hospitalizations, and emergency department visits.

· Coordinate resident admissions, transfers, discharges, and returns from hospital or other healthcare settings.

· Ensure resident service plans accurately reflect current needs and interventions.

· Ensure appropriate documentation is completed timely and accurately and participate in incident investigations and follow-up as assigned.

· Immediately report suspected abuse, neglect, exploitation, misappropriation, or other reportable events according to facility policy.

Daily Unit Oversight

· Complete routine rounds throughout the unit and ensure residents are appropriately supervised and their needs are being met.

· Monitor staff assignments and ensure adequate coverage; provide direction and support to caregivers and other staff assigned.

· Ensure staff follow resident service plans and individualized dementia-care approaches.

· Address resident, family, and staff concerns promptly and communicate operational concerns to the appropriate department leader.

· Promote teamwork among nursing, activities, housekeeping, and nutrition services.

· Ensure the unit maintains a calm, welcoming, organized, and resident-centered atmosphere.

· Identify and address immediate safety or environmental concerns and participate in shift-to-shift communication.

· Meaningful Moments activity program oversight

Dementia Care Leadership

· Model calm, respectful, and supportive approaches when interacting with residents.

· Coach staff regarding validation, redirection, cueing, reassurance, and other dementia-care techniques.

· Identify potential causes of expressions of distress or changes in behavior and evaluate for possible unmet needs.

· Promote non-pharmacological interventions whenever appropriate.

· Encourage staff to use Meaningful Moments activities as an intervention for anxiety, restlessness, loneliness, or other behavioral expressions.

· Monitor for changes requiring clinical evaluation and communicate effective resident approaches to the team.

Requirements

  • Success completion of Licensed Practical Nurse curriculum from an accredited LPN Program
  • Current Ohio LPN license in good standing
  • Demonstrated ability to provide clinical leadership and staff supervision
  • Knowledge of person-centered dementia care principles
  • Ability to coordinate multiple departments and priorities within a memory care environment
  • Strong communication, organizational, clinical judgement, and problem-solving skills
  • Demonstrates compassion, patience, flexibility, accountability, and leadership
  • Experience in assisted living, long-term care, geriatrics, or memory care preferred; dementia care experience strongly preferred
