Duties/Responsibilities
• Assist in the planning, development, organization, implementation, evaluation, and direction of assigned shifts’ activities.
• Meet with administration, medical and nursing staff, and related departments in planning department services, programs, and activities.
• Assist in development, implementation, and maintenance of nursing objectives and standards.
• Assist in development, implementation, and coordination of departmental policies and procedures, resident care plans, nursing procedure manuals, and job descriptions.
• Ensure that departmental personnel, residents, and visitors follow established policies and procedures at all times.
• Be involved in the planning and implementation of recommendations by the Care Plan Team, RISK Committee, Utilization Review Committee, and Medicare Stand Up meetings.
• Ensure that documentation of residents’ daily medical record is accurate, comprehensive, and timely.
• Receive, document, and implement physicians’ orders in an accurate and timely manner.
• Coordinate admit and discharge of residents to and from the Smith Care Center.
• Ensure timely response to resident requests/call lights.
• Coordinate Home Care services and Smith Care Center services as indicated.
• Ensure the proper recording and accurate count of schedule drugs at all times.
• Ensure security of locked Smith Care Center areas; monitor that only authorized staff is in possession of Smith Care Center keys.
• Ensure accurate and comprehensive reporting of resident condition via shift-to-shift reporting.
• Interpret departmental policies and procedures to personnel, residents, visitors, and family members.
• Visit each resident at least once during a shift to ensure appropriate care is being rendered and act accordingly.
• Ensure that Residents’ Bill of Rights is followed at all times.
• Ensure that residents have a clean, safe, orderly, and comfortable environment.
• Assist in the development of and participate in programs designed for in-service education. Monitor orientation for unlicensed new hires. Orient licensed new hires.
• Establish and maintain rapport with the medical profession and other health related facilities and organizations.
• Assist in identifying departmental problems and subsequent resolutions.
• Make recommendations to the Director of Health Services regarding staffing, improvement of services, and potential problem areas.
• Participate in discharge planning, development and implementation of resident care plans and resident assessments.
• Perform administrative duties as required by departmental policies and procedures and report to the Director of Health Services.
• Assist in coordinating transportation to other health care facilities and individuals as indicated.
• Participate in departmental projects and studies as assigned.
• Coordinate health services, i.e., admissions, discharges, etc.
• Ensure that a stock level of medications, medical supplies, equipment, and other supplies is maintained.
• Inspect nursing care service areas and practices regularly to ensure compliance with current applicable regulations.
• Cooperate with resident’s physician in planning nursing care; periodically reviewing and updating care plans with MDS Coordinator.
• Complete medical forms according to departmental policies and procedures.
• Notify the resident’s attending/alternate physician, family, or guardian, of any change in condition, accident, or incident according to established facility policy.
• In cases of death, implement arrangements according to resident’s, families, or guardian’s wishes and in compliance with facility policy.
• Evaluate emergencies, determine emergency measures to be used, and notify the Director of Health Services of the emergency.
• Assist in the standardization of methods in which work will be accomplished.
SUPERVISORY RESPONSIBILITY
• The LPN has supervisory responsibility for Certified Nursing Assistants.
Education
• Graduate of an accredited school of licensed practical nursing.
Experience
• Minimum one year in the field of Long-Term Care preferred.
General
• Must have good organizational skills.
• Must be a detail-oriented person and able to perform duties with great accuracy on a daily basis.
• Must have excellent personal skills, demonstrating the ability to conduct oneself in a non-controversial style; a style that inspires the respect of others and promotes a sense of trust in the individual’s competence.
• Must have good communication skills; speaking, writing, and listening.
• Must be fluent in English.
• Must have good computer skills with the ability to learn and use proprietary software as required.
CERTIFICATES/LICENSES/REGISTRATIONS
• Must have valid certification as a Licensed Practical Nurse in the State of Florida.
• Must have a current CPR (Cardiopulmonary Resuscitation) certification.
