  LTC/SNF/Assisted Living experience preferred.

  Strong clinical, communication, and care skills.

  Able to follow care plans and collaborate with staff.

  Clinical Care:
• 
  Assist with ADLs and monitor resident health status.
• 
  Administer medications and treatments.
• 
  Document resident care and report all changes.

• 
  Collaboration & Safety:
• 
  Follow physician orders and established care plans.
• 
  Promote a safe, clean, and supportive environment.
• 
  Provide emotional support and companionship.
