Job Title: LPN — Licensed Practical Nurse

Location: Wallingford, CT — On-site

Type: Full-Time | On-site

We are seeking Licensed Practical Nurses for full-time positions within a senior living and long-term care environment in Connecticut. LPNs will provide direct nursing care to residents under the supervision of a Registered Nurse, ensuring compliance with state regulations and facility standards.

Key Responsibilities

• Administer medications and treatments as directed by the plan of care
• Conduct routine patient assessments and document findings accurately
• Communicate changes in resident condition to supervising RN and physician
• Supervise and delegate tasks to CNA staff on assigned unit
• Participate in care planning, discharge planning, and family communication
• Maintain compliance with Connecticut state nursing regulations and facility policies

Requirements

• Active Connecticut LPN license required
• Minimum 1 year of long-term care or senior living experience preferred
• CPR/BLS certification required
• Strong clinical assessment and documentation skills
• Ability to work rotating shifts including evenings and weekends
