Job Description

The Flex Licensed Practical Nurse (LPN) is responsible for assisting in the planning, organization, development, and implementation of nursing services in accordance with established Plymouth Harbor policies and procedures, including the Plymouth Harbor Home Health Agency and support services, as directed by the Administrator of Home Care, ensuring the highest possible degree of quality resident care.
Shift: 12-hour shifts days or overnights.
DUTIES/RESPONSIBILITIES
1. Assist in the planning, developing, organizing, implementing, and evaluating of assigned activities.
2. Meet with administrative, medical, and nursing staff, in planning Home Care services, programs, and activities.
3. Implement and maintain nursing objectives and standards.
4. Assist in developing, implementing, and coordinating Home Care policies and procedures, resident care plans, nursing procedure manuals, and job descriptions.
5. Ensure that residents, family members, and visitors follow established policies and procedures at all times.
6. Ensure that resident rights to fair and equitable treatment, self-determination, individuality, privacy, and property and civil rights are respected.
7. Ensure that residents have a clean, safe, orderly, and comfortable environment.
8. Maintain rapport with other health related facilities and organizations.
9. Assist in identifying Home Care problems and subsequent solutions.
10. Make recommendations to the Administrator of Home Care regarding improvement of services and potential problem areas.
11. Participate in discharge planning, development, and implementation of resident care plans and resident assessments.
12. Assist in arranging transportation to other health care facilities/providers.
13. Participate in Home Care projects and studies as assigned.
14. Assist in ensuring that a stock level of personal medications, medical supplies, equipment, and recordkeeping supplies is maintained.
15. Assist in orienting new staff.
16. Cooperate with the RN Case Manager in planning nursing care, periodically reviewing and updating care plans.
17. Complete recordkeeping forms according to Home Care policies and procedures.
18. Report any changes in the resident’s condition to the RN Case Manager with the reports documented in the clinical record.
19. Provide care to the resident including the administration of treatments and medications.
20. In cases of death, implement arrangements according to Home Care policy.
21. Evaluate emergencies and determine emergency measures to be used.
22. Assist in standardizing methods in which work will be accomplished.
23. Notify the RN Case Manager verbally and in writing of accidents/incidents according to Home Care policy.
24. Collect laboratory specimens as ordered.
25. Perform prescribed monitoring, diagnostic, and therapeutic procedures.
26. Assist physicians as indicated.
27. Comply with Florida State statutes regarding required continuing education credits.
28. Work safely; complying at all times with Plymouth Harbor safety standards.
29. Attend and participate in workshops, seminars, and mandatory in-services as approved or required.
30. Maintain work area, equipment and supplies in a clean, sanitary and organized manner. Ensure that work areas are neat and confidential materials are properly stored before leaving area on breaks, end of work day, etc.
31. Discharge properly such other responsibilities and duties as the Director of Home Care Services may direct.
SUPERVISORY RESPONSIBILITY
• The LPN has supervisory responsibility for Certified Nursing Assistants.
QUALIFICATIONS
Education:
• Graduate of an accredited hospital diploma program or associate degree program for licensed practical nurses.
Experience:
• Minimum one year in a Home Health, Assisted Living, or Long-Term Care setting.
General:
• Must have good organizational skills.
• Must be a detail-oriented person and able to perform duties with great accuracy on a daily basis.
• Must have excellent personal skills, demonstrating the ability to conduct oneself in a non-controversial style; a style that inspires the respect of others and promotes a sense of trust in the individual’s competence.
• Must have good communication skills; speaking, writing, and listening.
• Must be fluent in English.
• Must have good computer skills.
• Must be proficient with Microsoft Office programs with emphasis on Excel, Outlook, and Word, with the ability to learn and use proprietary software as required.
CERTIFICATES/LICENSES/REGISTRATIONS
• Must have valid certification as a Licensed Practical Nurse in the State of Florida.
• Must have a current CPR (Cardiopulmonary Resuscitation) certification.
• Must have a valid Florida Driver’s license.

As required by Florida law (House Bill 531), candidates must complete a Level 2 background screening through the Florida Care Provider Background Screening Clearinghouse as a condition of employment.
