Location: Twin Cities Metro Area | Patient Visits Within Approximately a 35-Mile Radius of Your Home Schedule: Full-Time Compensation: $30–$38 per visit Benefits: Comprehensive Benefits Package + Mileage & Cell Phone Reimbursement To Stand Out Email Elena Ehrlich at elena.ehrlich@hhcare.net with your resume and a brief overview of your LPN experience, clinical background , and what interests you about providing nursing care in patients' homes. A Care Plan Can Look Perfect on Paper. Then You Walk Into the Patient's Home. The medication list says one thing. The pill organizer tells another story. The discharge instructions say the patient should be following a specific diet. Their refrigerator tells you what's actually happening. The treatment plan says the caregiver will help. Then you realize the caregiver has questions of their own. That's home health nursing. As an LPN Visit Nurse , you'll see something that's difficult to understand from a hospital room or nurses' station: How your patients are actually managing their health at home. And you'll have the opportunity to help make their care plan work in the real world. See the Patient. See the Environment. See the Whole Picture. Home health gives nurses a different perspective. You're not only asking whether a patient is taking their medication. You can help determine why they aren't. You're not simply providing wound care. You're seeing whether the patient or caregiver understands what needs to happen between visits. You're not just educating someone before discharge. You're helping reinforce that education after they've returned home. Sometimes the most important thing you discover during a visit isn't the reason you were originally sent there. That's where your nursing instincts matter. Your Workday Happens in the Community You'll travel throughout a territory centered around where you live, providing scheduled skilled nursing visits within approximately a 35-mile radius of your home. Instead of spending your entire shift inside one facility, you'll move throughout your local community. Drive to a patient. Provide the care they need. Educate the patient or caregiver. Document the visit. Then head to your next home. You'll have independence throughout your day while remaining connected to experienced RN Case Managers, Clinical Managers, physicians, therapists, and interdisciplinary clinicians. What You'll Help Patients Manage at Home Your visits may include: Wound care and dressing changes Medication management and education Skilled nursing observations Injections and ordered treatments Chronic disease monitoring Patient and caregiver education Reinforcement of physician orders and discharge instructions Monitoring for changes in condition Identifying potential safety or care concerns in the home Communicating clinical changes to RN Case Managers and physicians Coordinating with therapists and other members of the care team Accurate and timely clinical documentation Your goal isn't simply to complete the visit. It's to help make sure the patient's care continues working after you leave. Why LPNs Choose Home Health Meaningful 1:1 patient care Patient visits within approximately a 35-mile radius of your home Greater independence in your workday Opportunity to see the patient's complete home environment Build ongoing relationships with patients and caregivers Use your observation and critical-thinking skills Provide hands-on skilled nursing care Help patients successfully transition from hospital or rehabilitation back home Experienced clinical support available when you need it A Few Perks We're Especially Excited to Offer $30–$38 per visit Employer contributions toward medical insurance Employer-paid dental, vision, life insurance, and disability coverage Mileage reimbursement between patient visits Cell phone reimbursement for eligible clinicians Free access to our MedBridge Education Hub for continuing education and professional development Licensure reimbursement when it's time to renew Annual scrub reimbursement Three weeks of PTO plus six paid holidays Structured orientation, mentorship, and ongoing clinical support Career opportunities throughout our family of healthcare companies Your Experience Can Translate to Home Health You don't necessarily need to have spent your career driving from patient to patient. LPN experience in settings such as: Skilled nursing TCU Rehabilitation Assisted living Hospice Clinic nursing Other direct patient-care environments can provide an excellent foundation for home health. You already know how to assess. How to educate. How to recognize when something doesn't look right. How to communicate with patients and families. We'll help you learn how to bring those skills into the home. What We're Looking For Active Minnesota Licensed Practical Nurse (LPN) license Strong clinical observation and patient-care skills Ability to work independently and make sound clinical decisions Excellent patient and caregiver communication Strong organization and time management Compassionate, dependable approach to nursing Reliable vehicle, valid driver's license, and current auto insurance Ability to travel to patients within approximately a 35-mile radius of your home Home health experience preferred, but not required Why Home Health Care, Inc.? Take Your Nursing Beyond the Facility. See where your patients live. Understand what they're managing. Recognize the barriers. Teach the family. Provide the skilled care. Help turn the physician's orders into something the patient can actually follow at home. If you're an LPN who wants to see more of the patient's story , we'd like to meet you.
