Day Shift

We are seeking a compassionate, skilled, and patient-focused Licensed Practical Nurse (LPN) to join our Dialysis Department. Under the direction of a qualified Dialysis Registered Nurse, the Dialysis LPN provides direct patient care and supports the delivery of high-quality hemodialysis services. This position offers an opportunity to make a meaningful impact in the lives of patients while working within a collaborative and supportive healthcare environment.

The Dialysis LPN performs nursing tasks and responsibilities associated with the direct care of adult patients receiving dialysis treatment. The successful candidate will possess knowledge of renal disease processes and hemodialysis procedures and will assist in implementing therapeutic and preventive nursing measures as outlined in each patient's plan of care.

The Dialysis LPN is responsible for assessing pertinent clinical information, communicating observations to the Registered Nurse, contributing to patient care planning, providing health education and counseling, and performing advanced therapeutic measures within the scope of practice.

Essential Responsibilities

  • Provide direct patient care under the supervision of a qualified Dialysis RN.
  • Monitor patients during dialysis treatments and report clinical observations to the RN.
  • Assist in implementing individualized patient care plans.
  • Perform therapeutic and preventive nursing interventions within the LPN scope of practice.
  • Conduct patient and family health teaching and counseling.
  • Maintain accurate and timely documentation in the electronic medical record.
  • Adhere to established policies, procedures, and regulatory standards.
  • Promote patient safety, dignity, and well-being at all times.
  • Support a positive team environment through effective communication and collaboration.
  • Maintain confidentiality of patient and organizational information.

Qualifications

Required Education

  • Graduate of an accredited School of Practical Nursing.

Required Licensure & Certifications

  • Current New York State Licensed Practical Nurse (LPN) license.
  • Basic Life Support (BLS) certification through the RQI system during clinical orientation.
  • IV Therapy Certification after one year of nursing practice.

Required Experience

  • Minimum one (1) year of acute care nursing experience.

Preferred Experience

  • One (1) year of experience in:
    • Dialysis
    • Intensive Care Unit (ICU)
    • Coronary Care Unit (CCU)
    • Emergency Room (ER)
    • Operating Room (OR)

Skills & Competencies

  • Strong interpersonal, communication, and organizational skills.
  • Ability to manage multiple priorities and meet deadlines.
  • Demonstrated professionalism and excellent customer service skills.
  • Ability to maintain confidentiality and exercise discretion.
  • Computer proficiency and ability to work within electronic medical record systems.
  • Experience with Microsoft Office applications preferred.
  • Commitment to teamwork and positive role modeling.

Patient Population

The Dialysis LPN must demonstrate the knowledge and skills necessary to provide age-appropriate care and effectively assess and respond to the needs of adolescent, adult, and geriatric patient populations.

Work Environment

This position involves occupational exposure to blood, body fluids, and other potentially infectious materials. Appropriate safety procedures and personal protective equipment are required.
