Job Title: LPN
The LPN is responsible to provide direct care to individuals living in a residential setting. He or she will follow the outlined plan of care, identify any health-related needs, monitor medical status and report changes to the on-call Registered Nurse, and work with residential staff to support the individuals in their community residence. LPN will follow established standards of care and operate within their scope of practice as defined by the Massachusetts Nurse Practice Act.
Schedule: Sunday and Saturday 9:00 am-3:00 pm and 3:00 pm- 11:00 pm Par Time (32hrs)
