Position Type: Full Time
Job Shift: MorningThe Crisis LPN provides direct nursing care and supports the health and safety of individuals in a crisis residential setting. This role focuses on clinical responsibilities, medication administration, and shift based oversight of care delivery. The LPN is clinically supervised by a Registered Nurse. Administrative and operational management will be carried out by the Crisis Program Manager.
Essential Job Duties and Responsibilities
Clinical Nursing Care
- Provide skilled nursing care including health assessments and monitoring of individuals served.
- Implement physician and RN directed care plans.
- Observe and document changes in physical or behavioral condition.
- Report significant changes promptly to the RN.
- Support crisis intervention efforts from a clinical perspective.
Medication Administration and Monitoring
- Administer medications in accordance with physician orders and DBHDD standards.
- Perform and document medication counts, including narcotics, per policy.
- Monitor individuals for medication effectiveness and adverse reactions.
- Ensure accurate and complete Medication Administration Records(MAR)
- Communicate medication concerns to the RN, pharmacy or physician as appropriate
Shift Based Oversight of Care
- Serve as the clinical lead on assigned shift for nursing related matters.
- Ensure that care is delivered in accordance with established plans and clinical standards.
- Provide guidance to direct care staff regarding health related needs of individuals.
- Ensure proper shift handoff, including nursing report and medication reconciliation.
Health and Safety Responsibilities
- Monitor the environment for health safety risks and report concerns.
- Ensure infection control practices are followed.
- Respond appropriately to medical emergencies.
- Ensure individuals are in a safe condition from a clinical standpoint.
Documentation and Compliance
- Maintain accurate, timely and complete clinical documentation.
- Complete incident reports related to medical or health concerns within required timeframes.
- Ensure documentation meets DBHDD and agency standards.
- Participate in clinical reviews or audits as requested by nursing leadership.
Communication and Collaboration
- Communicate effectively with the RN regarding all clinical matters.
- Collaborate with interdisciplinary team members regarding health needs.
- Proved clear and accurate shift reports to incoming nursing staff.
Additional Responsibilities
- Perform other clinical duties as assigned within LPN scope of practice
- Maintain current licensure and required certifications
- Work is performed in office, program, and residential settings with a moderate noise level.
- While performing duties, the employee is frequently required to stand, walk, communicate, and reach with hands and arms.
- Occasionally required to sit, climb, stoop, kneel, crouch, crawl, or lift/move up to 50 pounds.
- Specific vision abilities include close, distance, peripheral, depth perception, and focus adjustment.
- Reasonable accommodations may be made to enable individuals with disabilities to perform essential functions.
Travel Required
- Position may require travel.

Supervisory Responsibilities
- This position directly supervises the activities of the Direct Support Professional and Registered Behavioral Technicians and agency staff. Carries out supervisory responsibilities in accordance with the organization’s policies and applicable laws. Responsibilities include interviewing, hiring and training employees for the crisis homes.

Qualifications
- Graduated from an accredited School of Nursing
- A minimum of at least two (2) years’ experience serving individuals with developmental disabilities.
- A minimum of three years’ recent experience working with individuals who have autism spectrum disorders and their families. The candidate should demonstrate strong interpersonal skills, the ability to implement supportive strategies, and a commitment to promoting independence and quality of life for the individuals served.
