Phoenix Home Care and Hospice is hiring an LPN Clinical Manager to lead and support our Home Health team in Independence. If you are the nurse everyone already turns to for answers, support, and problem solving, this is your opportunity to take the next step.

This role combines clinical leadership, field support, and care coordination in a fast paced home health environment where your experience and decision making truly matter.

What You Will Do

Support and guide field nurses and caregivers

Assist with care plans and clinical oversight

Conduct supervisory and client visits

Help ensure quality patient care and compliance

Communicate with physicians, families, and the care team

Support scheduling, training, and clinical problem solving

What You Need

Active Missouri or Compact LPN license

Home health experience preferred

Strong organization and communication skills
