We are seeking a dedicated Licensed Practical Nurse (LPN) Clinical Coordinator to join our growing team. In this role, you'll play a vital part in coordinating patient care, supporting clinical operations, and ensuring patients receive the right care at the right time—all while making a meaningful impact in the lives of those we serve.

Current LPN/LVN license
Minimum of 2 years of LPN/LVNexperience preferred
Home health experience preferred
Knowledge of Medicare and Medicaid regulations, benefits, and home health practices preferred
Strong organizational and time-management skills
Excellent communication and interpersonal abilities
Positive, patient-centered approach to care
Commitment to delivering exceptional clinical outcomes and service
