Job Description

LPN Clinical Admission Coordinator - per diem

VNA Care is seeking a per-diem LPN Clinical Admission Coordinator to join our Worcester team.

Reviews and coordinates all needed clinical information prior to patient admission to ensure the perfect pathway for new patients and to ensure an exceptional customer service experience for the patient. Communicates with branches and teams regarding new patients and maintains positive, professional relationships with all internal and external customers.

Compensation based on experience: $25.00 - $35.00

Schedule:

Per-diem, 8:00AM-4:30PM, weekdays, weekends, and holidays available

Current LPN License required.

Experience:

• Minimum 2 years related experience including medical/surgical and community health and/or experience with a managed care organization. Current LPN license required.

• Previous intake, liaison or discharge planning experience is preferred.

• Knowledge of home care standards and practices required.

• Knowledge of medical terminology and treatment modalities of patient diagnoses required.
