Position Type: Full Time
Boone Memorial Hospital is seeking a dedicated Chronic Care Management (CCM) Nurse (LPN) to support patients with chronic conditions through ongoing care coordination, education, and monitoring. This position focuses on improving patient outcomes by promoting self-management, facilitating communication between care teams, and providing consistent support to patients between office visits through telephonic and in-person interactions.
Essential Functions
Reasonable accommodations may be made to enable individuals with disabilities to perform these essential functions. The CCM Nurse conducts patient assessments to identify needs, goals, and barriers to care; develops, implements, and monitors individualized care plans; tracks patient progress and adjusts care plans as needed; and provides ongoing monitoring and support between office visits. The CCM Nurse addresses patient questions and concerns via telephone and collaborates closely with primary care providers and other healthcare professionals.
This role facilitates communication between patients, families, and providers; assists patients with timely access to medical and specialty care; and coordinates transitions of care between inpatient and outpatient settings. The CCM Nurse educates patients and families on chronic conditions and self-management strategies, provides encouragement and support, and assists patients in accessing community resources and services. Participation in quality improvement initiatives and maintaining current knowledge of best practices in chronic care management are also expected.

Qualifications, and Competencies
Strong communication and interpersonal skills, ability to work independently, strong assessment and care planning skills, ability to prioritize tasks and manage time effectively, knowledge of community resources and referral pathways, and the ability to use technology and electronic health records to manage patient care.

Physical Demands
Prolonged sitting and extended use of a computer.
Travel Required
Travel between clinics as needed.

Required Education and Experience
Licensed Practical Nurse (LPN) with an active license in good standing, experience working with patients with chronic conditions, and knowledge of electronic health records.

Preferred Education and Experience
Experience in chronic care management, care coordination, or an outpatient clinic setting.
