As an LPN Caregiver with KeystoneCare, you’ll play a vital role in helping elderly or convalescing patients live safely, comfortably, and with dignity in their own homes. You’ll provide hands‑on personal care and supportive services that promote independence while creating a clean, safe, and nurturing environment.

Our LPN Caregivers work in patient homes and care facilities, building meaningful connections with patients and families every day. You’ll collaborate closely with medical staff, nursing teams, and internal departments, functioning under the guidance of a Registered Nurse and our Scheduling Coordinator—never alone, always supported.

Primary Job Responsibilities

  • Providing frail, ill, or immobile clients with assistance, company, and comfort.
  • Executes the patient care plan and communicates well with RN Case Manager.
  • Monitoring the physical, mental, and emotional conditions of clients and reporting changes to designated parties.
  • Performing personal hygiene care
  • Performing light housekeeping duties in patient's living area only.
  • Is aware of and follows nutritional guidelines for patients.
  • Preparing meals and assisting clients with eating and drinking.
  • Assisting with the use of incontinence products or providing bathroom visit support.
  • Running errands, including shopping for groceries and basic household items for patient only.
  • Assisting clients from their beds into chairs or wheelchairs, and back.
  • With prior approval by Staffing Coordinator, accompanying clients on trips, such as medical appointments.
  • Maintaining a clean, tidy, and safe environment.
  • Documents care using Agency approved Electronic Visit Verification system from the patient's care location.
  • Consistently shows accountability to schedule and communicates all needs for scheduling changes to Staffing Coordinator.
  • With prior authorization and proper training, administering medicine and treatments as prescribed by healthcare providers.

Requirements

  • Ability to read, write, and communicate effectively
  • Ability to understand both written and verbal directions and instructions
  • Graduate of an accredited LPN program
  • Current LPN license (PA)
  • Successful completion of a job specific knowledge exam
  • Possession of strong organization skills
  • Understanding, compassionate attitude toward the care of the sick
  • Ability to document actions and observations with detail and accuracy
  • One year home health experience preferred.
  • Familiarity with Feeding Tubes, Assisted Breathing Devices (Ventilators) and Hoyer Lift
  • Employees designated in a driver status will be required to maintain a valid driver's license and auto insurance at all times.

KeystoneCare is a nonprofit organization that provides a full continuum of home health and hospice services in the Greater Philadelphia Region. KeystoneCare offers post-hospitalization home care, palliative care, comprehensive hospice services, private duty care, and telehealth services. For more than 20 years, we have helped improve the quality of life for our patients and their families with comfort and dignity, regardless of stage of life or state of health. KeystoneCare is also known as Keystone Hospice and KeystoneCare Home Health.

KeystoneCare is Medicare and Medicaid certified, and maintains contractual agreements with most of the major insurance plans in the region. We are also a participant in the Pennsylvania Department of Human Services'​ Aging Waiver Program. KeystoneCare is governed by a Board of Directors.

The Keystone Homecare and Hospice teams are comprised of social workers, certified nursing assistants, licensed practical nurses, registered nurses, nurse practitioners, medical director, physical therapists, occupational therapists, chaplain, and a certified music therapist. Keystone House, the residential hospice in the historic John Welsh House on Stenton Avenue in Wyndmoor, Pennsylvania, has a support staff that also includes volunteers from the local community.
