LPN Care Coordinator - Private Duty Home Care

The LPN Care Coordinator for Private Duty Services plays a vital role in ensuring high-quality, compliant, and efficient delivery of care to clients. This position supports the coordination of care services by managing scheduling, overseeing documentation quality, and maintaining communication between caregivers, clients, and clinic staff. The coordinator ensures that all care activities align with regulatory requirements and organizational standards.

Key Responsibilities:

  • Coordinate and support scheduling of nurses and aides to ensure appropriate coverage for clients receiving care.
  • Monitor and perform quality assurance checks on all clinical documentation entered into the EMR system by nurses and aides, ensuring accuracy, completeness, and compliance.
  • Track and follow up on physician orders, including 485 plans of care and authorizations, ensuring timely updates and approvals.
  • Provide coverage for open shifts when necessary to maintain continuity of care.
  • Answer and manage incoming phone calls, addressing inquiries and routing communication appropriately.
  • Assist with visit verification processes to ensure accurate and timely payroll preparation.
  • Support onboarding and orientation of new staff, including training on policies, procedures, and EMR usage.
  • Maintain ongoing communication with providers to support coordinated patient care and service delivery.

Requirements

Qualifications:

  • Active Licensed Practical Nurse (LPN) license in good standing.
  • Experience in home health or private duty care preferred.
  • Strong organizational and multitasking skills with attention to detail.
  • Knowledge of EMR systems and healthcare documentation standards.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and collaboratively in a fast-paced environment.
  • Flexibility to assist with scheduling and occasional shift coverage as needed.

Work Environment:
This position may involve a combination of office-based coordination and occasional field responsibilities, depending on staffing needs.

Physical Requirements:
Ability to perform typical nursing duties, including sitting, standing, and occasional lifting, as required for shift coverage.
