Here are some of the main functions of our LPN Care Coordinators:

  • Oversee care for patients transitioning from healthcare facilities, including follow-up calls and coordination with the Primary Care Provider-Team.
  • Coordinate care for at least 30 high-risk and rising-risk patients, including using risk stratification tools and managing registries.
  • Arrange follow-up visits, and manage patient data on symptoms, medication, and support.
  • Handle various clinical tasks including drawing up medications for clinicians to administer, and monitoring devices such as Ambulatory Blood Pressure, Continuous Glucose Monitoring, Retinavue for diabetic patients, while ensuring HIPAA confidentiality and pursuing ongoing professional development.
  • Handling incoming phone calls from patients, creating orders, delivering test results via telephone, medication refills, scheduling appointments, calling outside medical facilities for scheduling of specialty appointments and test orders.
  • An eligible candidate should have the following minimum qualifications:
    • An active Licensed Practical Nurse license
    • Basic Life Support (BLS) certification required

*The starting rate of compensation for this position is $30.00 per hour. LPN Care Coordinators also receive an additional $1.50 per hour shift differential for working during weekends and holidays, as part of the rotation schedule.*

Requirements

  • Meet The Wright Center for Community Health and its affiliated entity The Wright Center for Graduate Medical Education EOS© People Analyzer Tool
  • Buy in and experience working in the EOS® model (strongly preferred)
  • Mission-oriented; represents the enterprise in a professional manner while demonstrating organizational pride
  • Graduate of an accredited LPN program
  • Active Pennsylvania Practical Nursing license
  • Previous experience in a health center setting a plus
  • BLS Certification
  • Experience with electronic health record, Medent a plus
  • Ability to work as part of a care team
  • Excellent communication skills
  • Commitment to process improvement and quality

*This position requires employees to participate in the following rotation schedule to meet the needs of patients and families:

  • Evenings (one late night per week; 12PM-8PM)
  • Weekends (either one Saturday or Sunday per month; when worked, employees will have a day off during the week)
  • Holidays (one to two holidays per year)
