Job Description

Bring compassionate hospice care to the heart of our senior living communities.

Join our team as a Hospice LPN and provide skilled, compassionate care to hospice patients in senior living communities in or around Albemarle County. Working alongside an RN, you’ll build trusted relationships with patients, families, and community care teams while helping ensure every patient receives comfort, dignity, and exceptional end-of-life care.

Responsibilities/General Duties

  • Deliver nursing services per Hospice policies
  • Document care using EMR
  • Assist physicians/RNs with procedures
  • Administer medications/treatments as prescribed
  • Observe, record, and report patient conditions to RN
  • Follow Hospice Plan of Care for patient/family services
  • Report changes in patient needs to RN case manager/team leader
  • Explain Hospice services to patients and families
  • Communicate with Interdisciplinary Team for coordinated care
  • Participate in QA programs and required meetings
  • Engage in professional development and maintain affiliations
  • Maintain patient confidentiality

Qualifications

  • Licensed as a Licensed Practical Nurse in the Commonwealth of Virginia
  • Two years’ relevant nursing experience preferred
  • Ability to manage all aspects of terminal care, health team service coordination, and documentation
  • Basic to intermediate proficiency in word processing software
  • Driver’s license, personal automobile, and current automobile liability insurance

  • The employee must occasionally lift and/or move up to 50 pounds and push up to 100 pounds in a wheelchair.
  • Physical and emotional health are necessary for the accomplishment of position duties.
  • Frequent travel may be required
