LPNs are responsible for providing skilled nursing care to participants. This position is responsible for working directly with the Interdisciplinary Team (IDT), participant, and his or her representative to achieve the goals of the care plan. Communicates to the family or caregiver and provides appropriate patient education. Schedule offered is Sunday-Wednesday 8:00 AM to 6:30 PM and is subject to change. A shift differential premium is provided for weekends.

RESPONSIBILITIES AND DUTIES:

ESSENTIAL JOB FUNCTIONS:

  • Performs skilled nursing care procedures such as ear lavage, wound care, fingerstick for blood sugar/INR, administration of oral/topical/injectable/IV medications and immunizations, fills medication planners, measures for and applies compression garments, and other procedures as assigned and according to licensure regulations
  • Provides patient education related to medications, inhalers/nebulizer treatments, and point of care testing such as fingerstick for blood sugar
  • Performs nursing follow up visits after ER visits and falls as assigned
  • Performs lab sample collection functions for participants when necessary. Identifies and labels specimens appropriately. Handles potentially infectious specimens with appropriate biohazard precautions, and practices universal precautions. LPN II: performs phlebotomy as assigned
  • Documents assessments, care, and care coordination in the EMR
  • Implements home care nursing orders written by medical providers to completion
  • Provides medication reconciliation and communicates with facilities and the pharmacy
  • Communicates effectively to the IDT and to family and caregivers, including providing instructions regarding medications and treatment
  • Observes, records, and reports participant’s condition and reaction to drugs and treatments to medical providers, using SBAR format
  • Other duties as assigned



Qualifications

  • Graduate of an accredited Licensed Practical Nurse Program and licensed as a professional in the State of Colorado is required
  • One year of health care experience in hospital, long-term or home health care setting with 1 year of experience with a frail or elderly population is preferred
  • Ability to perform phlebotomy functions
  • Knowledge of medical equipment and instruments
  • Must have excellent written and verbal communication skills and demonstrate the ability to interact clearly and effectively with both internal and external customers
  • Must have reliable transportation and maintain current automobile insurance
  • Must be efficient with computer applications such as email, navigating the internet, using electronic timekeeping, and Electronic Health Records
