The Licensed Practical Nurse (LPN) at Lower Lights Christian Health Center supports high-quality, patient-centered care by performing direct clinical services, gathering patient information, administering medications and treatments, managing provider in-basket in EHR and assisting providers during patient visits. Working as part of an interdisciplinary care team, the LPN helps ensure smooth clinic operations, supports chronic disease and preventive care initiatives, and contributes to a welcoming and efficient patient experience. This role requires strong clinical skills, attention to detail, and a commitment to LLCHC’s mission of delivering compassionate, whole-person healthcare to all individuals, regardless of ability to pay.

Licensed Practical Nurses are expected to understand and follow the Ohio Nurse Practice Act as administered by the Ohio State Board of Nursing and practice according to their nursing scope.

KEY RESPONSIBILITIES:

  • Performs focused health assessment of patients by completing the nursing process.

  • Identifies actual and potential health needs based on medical, mental/emotional, financial, and social factors.

  • Completes all interactions in a timely, professional manner with courtesy and respect.

  • Accepts responsibility for the interactive process that occurs over the telephone with the patient.

  • Provides care for patients using LLCHC-approved nurse clinical protocols.

  • Schedules appointments as the patient's condition requires, answer questions, and provides health education in relation to the call as needed.

  • Accurately, concisely, and succinctly documents all patient contact in the electronic medical record.

  • Takes lead in managing electronic health record inbox messages including patient calls, MyChart messages, refill requests, test result notifications, and other messages that directly impact patient care plans. LPN accurately sorts messages and either addresses or delegates to appropriate clinic team member.

  • Assist patients to establish short and long-term goals to obtain optimum level of health through advocacy, counseling, teaching, problem solving, and utilization of resources.

  • Monitors, plans, implements, and evaluates coordination and management for patients with chronic illnesses in collaboration with care team.

  • Monitors patient health maintenance and care gap closure opportunities in collaboration with clinical team.

  • Implements required nursing procedures consistent with LLCHC procedures/protocol.

  • Participates in process and quality improvement working groups or workflow optimization projects as needed.

  • Assist in new employee orientation, onboarding, and training as needed.

  • Performs other duties as assigned, including supporting medical assistant education and clinical quality.

  • Administers medications, immunizations, POC testing, ECGs, and other primary care procedures as requested by providers or based on LLCHC clinical protocols.

  • Active participate in team-based care model, including participation in daily huddles and panel or registry reviews.

  • Actively participates in the management of the clinic vaccination and injection programs in collaboration with clinical leadership.

Qualifications

QUALIFICATIONS:

  • Completion from an accredited license practical nurse program required.

  • 1-3 years related experience and/or training, preferably in a Federally Qualified Health Center (FQHC) or primary care field.

  • Previous EPIC or EHR experience is preferred.

  • A current, unrestricted Ohio LPN license required.

  • Active BLS certification required.

  • Ability to fluently speak, read, and write in Spanish preferred but not required.

WORK ENVIRONMENT:

  • The noise level in the work environment is usually moderate to loud based on patient volume.

  • As a result of high patient volume, this position experiences stressors that come along with a fast-paced environment.

  • Must be able to travel during the workday and/or cover at other locations as needed.

  • Position requires work in a clinical environment with potential exposure to blood-borne pathogens, hazardous chemicals, and contaminated medical instruments. PPE and clinical safety guidelines must be followed to ensure staff and patient safety.
