
Reports To: Director of Long Term Care
FLSA Classification: Full-Time, Non-Exempt, Hourly $35.00- $50.00
Essential Functions:
- Assists the Director in directing the activities of licensed and non-licensed personnel who provide healthcare and nursing services in the Walbridge Memorial Wing on a 24- hour, 7-day per week basis.
- Notify the Director as appropriate and necessary for staff and resident issues.
- Responsible for coordinating the CNA class.
- Ensure that necessary equipment checks are being performed.
- Assist the Director assuring that Long Term Care unit is compliant with all mandatory regulations, requirements, and audits.
- Assist the Director with coordinating and providing staff education and in-services.
- Assist the Director with assessing the educational needs of staff.
- Assist the Director in preparing annual facility objectives and goals.
- Assist the Director with completing Quality Improvement studies.
- Assist the Director with preparing for the annual State Nursing Home Survey.
- Assist the Director with developing the budget; using the budget to maintain staffing; documenting the need for additional equipment and supplies; and reviewing protocols, procedures, and guidelines.
- Assist the Quality Assurance and Performance Improvement (QAPI) committee in developing and implementing appropriate plans of actions
- Assists the Director in the development, implementation, and evaluation of policies, procedures and guidelines on an annual and ongoing basis.
- Assist the Occupational Therapy Aide/Restorative CNA with evaluations of residents and exercise programs.
- Monitor documentation in and completion of chart notes, medication administration record; conduct resident chart audits to ensure accuracy and completeness of resident’s medical record.
- Assist floor nurses during high volume times.
- Assist Charge Nurse with day-to-day management issues of Long Term Care, if needed.
- Orient new nurses to the Walbridge Wing.
- Orient new nurses to the PCC system.
- Assist MDS Coordinator as needed.
- In the absence of the Director, the Assistant Director of Nursing Services performs the duties of the Director.
- Other duties as assigned. Requires taking call and working extra shifts during high volume/census, low staffing, or emergent periods.
Education of Experience:
- High School Diploma or equivalent required
- Bachelor’s Degree preferred but not required.
- Current, unencumbered RN or LPN license required. If LPN, RN license required within three (3) years of hire.
- BLS required within four (4) months of hire or transfer.
- Three years’ experience in a long term care setting with one year management experience; or five years’ experience in a long term care setting required.

Pioneers' history began in 1947 when Freeman Fairfield, a previous resident of this community, approached the County Commissioners about establishing a hospital. Over the next several years, Mr. Fairfield worked with the Commissioners and a group of local citizens to build a hospital for the Meeker community. Opening its doors in 1950, Pioneers began offering acute health care and surgical services to the community, licensed through the State of Colorado Department of Health and Human Services. We are Medicare and Medicaid-certified, accept private insurance, and, of course, self-pay. Financial arrangements may be set up through the Business Office.
In 1964, the Walbridge Memorial Convalescent Wing opened its doors for long-term care, made possible through the generosity of the wills of Carl and Adelaide Walbridge. Attached to the north side of the hospital, the Wing, with its 33 beds, brings skilled and intermediate nursing to residents who are unable to care for themselves. Through the Wing's rehabilitation services, exercise, and activity groups, each resident's care is individualized to meet his or her physical, emotional, nutritional, and spiritual needs. The Wing is licensed by the State of Colorado Department of Health and Human Services, and accepts Medicare, Medicaid, and private-pay residents.
First-line health care services are delivered through Meeker Family Health Center. The MFHC originated in 1990 as a rural health clinic, and its physicians, nurse practitioners, and staff continue to strive to meet our community's and visitors' healthcare needs.
