Status: Full-Time

Position Summary

The LOA program director is responsible for overseeing program operations, driving census growth, managing facility engagement, and cultivating strategic referral relationships across key health systems. This role serves as the primary liaison between participating facilities, referral partners, and program leadership to ensure successful program execution, consistent growth, and high-quality partner engagement.

The ideal candidate is a proactive relationship builder with strong healthcare business development experience, contract negotiation skills, and a proven ability to increase patient volume through strategic partnerships and operational oversight.

Key Responsibilities

Program Management & Oversight

  • Navigate and oversee all aspects of the program to ensure operational success and compliance with program requirements.
  • Monitor facility participation, engagement, and performance metrics.
  • Act as the primary point of contact for participating facilities and hospitals.
  • Identify and address operational barriers impacting referrals, admissions, and overall program performance.
  • Provide regular updates and recommendations to leadership regarding program growth opportunities and challenges.

Interdisciplinary Team (IDT) Coordination

  • Attend and actively participate in all weekly IDT meetings.
  • Collaborate with clinical, operational, and administrative teams to ensure alignment on patient care and program objectives.
  • Track action items and facilitate communication between facilities and hospitals to promote successful outcomes.

Business Development & Relationship Management

  • Build and maintain strategic relationships with:
  • Memorial Hermann Health System
  • Harris Health System
  • Houston Methodist
  • Additional referral sources, providers, and healthcare organizations
  • Develop and implement referral growth strategies to increase program awareness and utilization.
  • Conduct routine visits with hospital leadership, discharge planners, case managers, physicians, and community partners.
  • Serve as an ambassador for the program within the healthcare community.

Contract Negotiation & Network Expansion

  • Negotiate individual facility and LOA levels to support program growth and sustainability.
  • Identify opportunities to expand the program's network footprint and market presence.

Census Growth & Performance Management

  • Maintain and grow Average Daily Census (ADC) from:
  • Current ADC: 20 participants
  • Short-term goal: 30 participants
  • Mid-term goal: 35 participants
  • Long-term goal: Continued sustainable growth beyond 35 participants
  • Establish and monitor referral pipelines and conversion metrics.
  • Develop action plans to achieve census and revenue growth targets.
  • Analyze referral trends and identify opportunities for increased market penetration.

QualificationsQualifications

Required

  • Bachelor’s degree in healthcare administration, Business, Marketing, Nursing, Social Work, or related field.
  • 3+ years of healthcare business development, provider relations, managed care, or program management experience.
  • Experience working with hospitals, health systems, SNFs, physician groups, or post-acute care providers.
  • Strong negotiation and contract management skills.
  • Excellent communication, presentation, and relationship-building abilities.
  • Ability to work independently and manage multiple priorities.

Preferred

  • Experience with PACE, value-based care, managed care, senior healthcare, or population health programs.
  • Existing relationships within the Houston healthcare market.
  • Knowledge of Memorial Hermann, Harris Health, and Houston Methodist referral networks.

Key Performance Indicators (KPIs)

  • Average Daily Census growth from 20 to 30, then 35+ participants with 60,90, 120 days
  • Number of new referral partnerships established.
  • Referral conversion rates.
  • Facility participation and engagement metrics.
  • Contract execution and network expansion goals.
  • Partner satisfaction and retention.
