Rotating ShiftMedical social worker in the acute hospital with the ability to work in multiple settings including ICU, med/surg. Emergency Department, mother/baby, and the psychiatric medical unit.

Work Location: Trinity Health St. Mary’s Hospital, Grand Rapids, MI

In this role, you will conduct psychosocial assessments, provide brief solution-focused counseling, and develop collaborative treatment goals. Your efforts will help patients overcome barriers, access community resources, and ensure continuity of care across inpatient and outpatient settings.

What You'll Do:

  • Conduct comprehensive psychosocial assessments for patients and families.

  • Develop treatment plans that address emotional, social, and resource-related needs.

  • Provide brief, solution-focused counseling and facilitate discharge or care planning.

  • Advocate for patients and coordinate referrals to community-based services.

  • Collaborate effectively with interdisciplinary care teams.

  • Document all assessments, plans, interventions, and outcomes clearly and promptly.

  • Meet departmental productivity expectations and performance standards.

What You'll Bring:

  • Education: Master’s Degree in Social Work or a related field from an accredited institution.

  • Licensure: Must be eligible for licensure as a Limited Licensed Master Social Worker (LLMSW) or Licensed Master Social Worker (LMSW) by the Michigan Board of Social Work.

  • Experience: Minimum of 2 years of social work experience in an acute inpatient setting.

  • Strong psychosocial assessment skills and clinical judgment.

  • Ability to navigate complex family dynamics and high-acuity situations.

  • Excellent crisis support and problem-solving abilities.

  • Outstanding interpersonal and empathetic communication skills.

  • Proficiency in engaging patients and care teams in effective treatment planning.

  • Knowledge of community resources and continuity of care principles.

  • Accurate, timely documentation in medical records and reporting systems.
