Amity Health is seeking a motivated Link Worker to join our team and support participants to access community groups, programs, and services that enhance social engagement and quality of life.
As a Link Worker, you will play a vital role in supporting participants to identify their goals and social needs, and connecting them with appropriate services, programs, and networks. Your responsibilities will include:
