The YAS Community Life Skills Coach provides one-to-one, community-based support to young adults living with significant mental health and/or co-occurring disorders. The Life Skills Coach promotes independence, daily living skills, community involvement, natural support development, and achievement of service plan goals.
ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.
• Provide face-to-face, community-based life coaching services tailored to individual service plans.
• Support individuals with daily living tasks including hygiene, nutrition, shopping, budgeting, and household maintenance.
• Help individuals build coping strategies, problem-solving skills, and self-management skills.
• Assist individuals with engaging in employment, education, cultural, spiritual, recreational, and social activities.
• Support access to medical, behavioral health, wellness, and community resources.
• Facilitate community integration and natural support development.
• Monitor behavior, symptoms, and environmental factors for safety.
• Review and implement service/recovery plans and collaborate with clinical teams.
• Complete timely documentation including notes, logs, incident reports, and other required records.
• Maintain utilization requirements and support performance outcome measures.
• Provide transportation for appointments and community activities.
• Follow safety protocols, CPR/First Aid guidelines, and crisis management procedures.
GENERAL DUTIES
• Maintain a safe and clean work environment and ensure that agency property is well maintained.
• Follow the Marrakech procedures, protocols, and Policies and Procedures manuals as well as conform to the work culture and expectations of DMHAS.
• Attend staff meetings and required in-service training.
• Operate motor vehicles safely and in accordance with State Law (agency and personal automobiles) while on work related duties.
• Report unsafe conditions and situations immediately per agency policy and procedures
• Serve all customers with alacrity.
• Communicate with co-workers and other providers accurately and professionally.
• Represent the values, mission and vision of the organization.

• Mental Health and Co-Occurring Disorder Awareness: Demonstrates foundational knowledge of mental health and co-occurring disorders, recognizing signs and symptoms and responding with appropriate support, boundaries, and referrals when needed.
• Skill Development and Coaching: Provides hands-on coaching in daily living skills (e.g., self-care, budgeting, time management, communication), using practical, real-life strategies to build confidence and independence.
• Community Integration and Resource Navigation: Supports individuals in accessing and navigating community resources, services, and opportunities, promoting meaningful participation in work, education, and social activities.
• Cultural Humility and Inclusion: Approaches all interactions with openness, respect, and cultural awareness, honoring diverse identities and experiences while creating inclusive, affirming environments.
• Strong Communication, Organization, and Problem-Solving Skills: Communicates clearly and professionally, maintains accurate documentation, manages time and priorities effectively, and applies critical thinking to address challenges and adapt support strategies.
QUALIFICATIONS

EDUCATION and/or EXPERIENCE
Bachelor’s degree in human services, counseling, social work or related fields is preferred along with experience supporting individuals with mental health or co-occurring disorders. A high school diploma/GED and two years’ experience supporting individuals with mental health or co-occurring disorders will be accepted in lieu of a degree. Ability to learn and practice a trauma-informed and responsive approach is required.
CERTIFICATES, LICENSES, REGISTRATIONS
• A Valid State of Connecticut Public Service Driver’s License is required.
• CPR and First Aid certification
Pay Type:
Hourly: $18.94/hr
Schedule:
15 Hours
Tuesday: 9a-2p, Wednesday: 10a-3p, Thursday: 10a-3p
