Life Enrichment Worker, Mobile (MLEW) is responsible for working with people in the community with complex mental health and physical health challenges in Simcoe County area. These complex challenges could include mental health and /or addictions challenges, cognitive impairments, ongoing physical health issues, responsive behaviours and/neurological disorders.
The MLEW is responsible for the operation and delivery of all recreation programming which will include assessing, planning, implementing, coordinating and evaluating all interventions.
Program delivery will follow four key streams:
The MLEW is a flexible mobile model that bring meaningful connections directly to isolate seniors. Through personalized support group and community access, these programs reduce isolation and builds lasting belonging to the community. This position will include both individual and group work.
Back to Home Penetanguishene provides specialized support to vulnerable and at-risk older adults with complex physical health challenges, mental health challenges, addictions or cognitive impairment LOFT responds to each resident’s needs, so they can live safely and in good health in the community as they age. Through our care, we aim to postpone for as long as possible, or eliminate, any need for institutional care. Our programs offers 24/7 care through our specialized Personal Support Worker (PSW) team. We provide services assistance, medication administration with personal care, essential housekeeping, laundry services, and escorts to appointments. LOFT staff receive enhanced training related to mental health, addictions, psychosocial rehabilitation, responsive behaviours, dementia and the impact of the social determinants of health, thus giving them specialized skills required to provide the highest quality of care to the populations we serve.
Back To Home Barrie is a supportive program for older adults facing complex challenges who are able to live safely in the community with access to subsidized housing and enhanced assisted living services. Our program specializes in supporting vulnerable older adults with complex physical and mental health challenges, addiction and/or cognitive impairment. Back To Home Barrie program is specifically designed to respond to each resident’s needs, so they can live safely and in good health in the community as they await for permanent housing and support. This program is a partnership with Simcoe Terrace Retirement Home, in which LOFT provides specialized personal support and one-on-one support services. Our program offers 24/7 care through our specialized Personal Support Workers (PSW), providing such services as assistance with personal care, essential housekeeping, laundry services, medication, support, escorts to appointments and medical assessment and treatment.
