We have opening for a LVN Intake Coordinator in our Needs Assessment Department.
Charter Oak Hospital offers compassionate and individualized treatment for mental health and substance use disorders. We believe in a patient-centric approach, where every treatment plan is tailored to meet the specific needs of each person who walks through our doors.
The Intake Coordinator assists in collecting patient information for possible admits, pre-certs, and filing mandatory reporting; take phone calls into the assessment center and routes calls as necessary; UR Pre-certification with insurance companies; Assist with taking physician orders and other duties as assigned.

Aurora Charter Oak Behavioral Health Care is located on 10 acres of beautifully landscaped grounds in Covina, California. We have been providing mental health and substance abuse treatment services at this same location since 1941. Covina is about 22 miles east of downtown Los Angeles, in the San Gabriel Valley. The San Gabriel Mountains provide a stunning backdrop to our campus, which includes Aurora Charter Oak Hospital and the Charter Oak Recovery Center. Our campus includes three separate buildings featuring eight separate units for adolescents and adults requiring inpatient care, a Residential Treatment Center (RTC), an Outpatient Services building, and an Assessment Center which is open 24 hours a day, 7 days a week. Our campus also includes a pool and a gymnasium.
