About the Role:

ComForCare Home Health Care (North San Diego) is looking for a compassionate and skilled Licensed Vocational Nurse (LVN) to join our dedicated home health team in San Diego, CA. This is a rewarding opportunity to make a real difference in patients' lives by delivering personalized, one-on-one care right in the comfort of their homes. Responsibilities:

  • Provide direct patient care in home settings, including wound care, medication administration, and vital sign monitoring
  • Develop and follow individualized patient care plans in coordination with the supervising RN and care team
  • Observe, document, and report changes in patient condition to appropriate clinical staff
  • Administer prescribed medications, injections, and treatments per physician orders
  • Educate patients and family members on care procedures, medications, and health management
  • Maintain accurate and timely clinical documentation in compliance with state and agency standards
  • Coordinate with multidisciplinary care teams to ensure continuity and quality of patient care

Requirements:

  • Active Licensed Vocational Nurse (LVN) license in the state of California
  • Current CPR/BLS certification
  • Prior home health, skilled nursing, or clinical care experience preferred
  • Strong knowledge of wound care, medication management, and patient assessment
  • Excellent communication, time management, and organizational skills
  • Reliable transportation and valid California driver's license with proof of insurance
  • Compassionate, self-motivated, and able to work independently in a home care environment
