Heritage Health Network (HHN)
is seeking a
Licensed Vocational Nurse (LVN)
to join our Enhanced Care Management (ECM) team supporting members within the
IEHP Pod
. The LVN will work closely with RNs, Lead Care Managers, Community Health Workers, Behavioral Health Care Managers, and the Director of Clinical Operations to provide coordinated, whole-person care.

This is a
member-facing role
that combines clinical support, care coordination, community-based member engagement, and clinical documentation.

What You’ll Do

• 
  Provide clinical support and care coordination for an assigned caseload of ECM members within the IEHP Pod.
• 
  Support individualized, person-centered care plans and address barriers to care, including avoidable ER and hospital utilization.
• 
  Conduct member visits in the community or home and accompany members to medical appointments as needed.
• 
  Complete risk assessments and collaborate with RNs and Lead Care Managers to develop and update care plans.
• 
  Use motivational interviewing to engage members, encourage adherence, and support behavior change.
• 
  Coordinate with hospitals and other providers on discharge planning and transitional care.
• 
  Review clinical documentation and care plan notes for accuracy and completeness.
• 
  Participate in daily pod huddles, case conferences, and multidisciplinary care reviews.
• 
  Help members access appropriate healthcare services, resources, and community support.
• 
  Maintain accurate, timely, and compliant documentation in ECW.
• 
  Ensure documentation supports health plan, regulatory, billing, and payment requirements.
• 
  Escalate cases outside the LVN scope of practice to the RN or Director of Clinical Operations.
