Job Description

The Staff Pad is proud to partner with a reputable health center in Pomona, CA committed to providing access to high-quality healthcare while empowering patients, employees, and community partners to improve their well-being and strengthen the health of the communities they serve. We are looking to hire a qualified and compassionate Licensed Vocation Nurse (Floor) for their Primary Care Department. Reporting to the Director of Operations, the LVN under the direction of a physician, advanced practice provider, or Registered Nurse, delivers compassionate patient care and works collaboratively with the healthcare team to promote health, prevent illness, and support positive patient outcomes. Schedule: Full-Time | 1.0 FTE What You’ll Do Key Responsibilities Demonstrate the health center’s mission by providing quality care and serving as an LVN resource to the care team, as applicable Perform nursing duties within scope, including wound care, immunizations, and patient health education Complete medication reconciliation and administer prescribed medications and treatments per written or standing orders Maintain up-to-date emergency kits and oxygen tanks Complete required forms in accordance with health plan and program standards Performs all duties in support of successful EHR and patient portal management Contact patients to schedule appointments as needed Participate in care team and other assigned meetings Deliver services in full compliance with the health center’s standards Respond to and appropriately escalate provider and patient messages using SBAR and established health center policies and procedures Communicate through messages with external providers and agencies regarding patient care, issue resolution, and escalation Respond to patient portal requests for forms, labs, and test results within scope Perform other related duties as assigned What We Are Looking For License, Certification and Experience High school diploma or equivalent Current, valid California LVN license BLS certification required 1+ year of LVN experience; outpatient clinic experience preferred Additional Skills and Attributes Ability to work effectively in a fast-paced environment Excellent written and verbal communication skills Professional, supportive, and team-oriented attitude Bilingual English/Spanish preferred What You Can Expect Work Environment Fast-paced, collaborative clinical environment Regular interaction with patients, providers, and healthcare staff Shared office and patient-care setting Frequent communication, documentation, and patient support Ability to manage multiple priorities while maintaining quality care Compensation and Benefits: Competitive Pay Medical, Dental and Vision Insurance Holiday Vacation and Sick Leave Employee Assistance Program Life Insurance (Basic + Voluntary) Tuition Reimbursement 403(b) Retirement plan with 6% match If you’re a dedicated LVN looking to grow your career while making a meaningful difference in patients’ lives, we invite you to apply and join a team that values your skills, compassion, and commitment to quality care.
