HealthRIGHT 360 offers residential treatment services designed to serve individuals and families with substance use disorders and co-occurring mental health disorders. Our program helps clients address addiction and related issues, increase skills, manage stressors, connect to supportive service, and build a foundation for lasting recovery. Residential programs run 24/7 and Individual and group therapy, family therapy, individualized case management, treatment planning, drug and alcohol counseling, Medication support services, and crisis intervention.

The required work schedule for this position is: 7:00AM - 3:30PM Sunday - Thursday.
HealthRIGHT 360, a nonprofit organization and a family of programs, is committed to providing accessible and comprehensive healthcare services to vulnerable populations. Our mission is to tackle systemic barriers to healthcare and promote health equity for all. We offer a wide range of services, including mental health care, residential and outpatient substance use treatment, and primary health services. Additionally, we provide transitional support for individuals re-entering the community after involvement in the criminal justice system. By integrating physical and behavioral health, we empower individuals to overcome challenges by addressing social determinants of health, fostering resilience, and facilitating recovery.
The Residential Step-Down Programs (RSD) provide people with transitional housing during their recovery from Substance Use Disorder (SUD) for up to one year. Clients are supported by a team of Recovery Coaches and Outpatient Services staff such as SUD counselors and behavioral health clinicians and they receive counseling and education for substance abuse, life skills, as well as individual and group therapy. Our goal is that our clients graduate with the skills and resources to sustain a steady income, affordable housing, and optimal well-being.
The Licensed Vocational Nurse (LVN) is an integral part of this interdisciplinary team that supports our clients in navigating evidence-based treatment plans and the healthcare system. The LVN collaborates with Registered Nurses, Health and Wellness staff, as well as other internal and external providers invested in the client’s health to provide treatment plan oversight and care coordination. They support treatment adherence via client education within their scope that assists them in developing strategies for taking their medication regularly and effectively. The LVN reports concerns related to adherence, medication interactions, client presentations, and/or the environment of care to the Registered Nurse and/or Medical Director as needed. This role utilizes the Nursing Process, Harm Reduction, and Trauma Informed Care modalities in all actions taken to advocate for and support the client in their planning and goals.
Leadership Responsibilities:
• Works collaboratively with the Health and Wellness staff.
• Provides clinical leadership for client health education, client medication support, and within the quality improvement infrastructure of the healthcare and residential program.
• Participates in matters related to performance and quality improvement, planning, protocols, and goal setting.
• Comfortable in changing systems, champions change, and lean process change.
Organizational Responsibilities:
• Proper and consistent documentation of required information accurately on Clinic records and reports. Keeps up to date with operational and procedural requirements.
• In conjunction with Director of Addiction Medicine, assures organizational readiness for accreditation surveys and ongoing monitoring and reporting of conformance to quality within the clinics.
• Supports the interpretation of standards and processes to meet best practice standards established in the health services program.
• Coordinates and consults with the appropriate department heads within the agency to facilitate integration between programs.
• Provides direct care coordination support for clients to access all aspects of HR360 healthcare services.
• Serves on appropriate committees and work groups and other related committees of agency.

QUALIFICATIONS
Education, Certification, and Experience
• Active CA Vocational Nursing License.
• First Aid and CPR certification.
• Experience and interest in working with safety-net populations and in treating substance use disorders.
• Experience working successfully with issues of mental health, criminal background, and other potential barriers to economic self-sufficiency.
Knowledge
• Knowledge of electronic medical records.
• Knowledge of co-occurring disorders and trauma informed treatment.
• Knowledge of clinical documentation (treatment plans, progress notes etc.).
