M-F 40 hours
Salary: $65,000 to 75,000 (Depending on licensure)

OF PRIMARY RESPONSIBILITIES:

The OnTrackNY Program is a Coordinated Specialty Care (CSC) treatment program at site name here serving adolescents and young adults who are within two years of experiencing the onset of psychosis. Our mission is to keep young people on track with their goals for school, work and personal relationships. Equity, inclusion, rapid access and self-determination are at the core of everything we do.
OnTrackNY team staff receive expert training and technical assistance in the CSC model and protected time for professional development through the statewide OnTrackNY Initiative at the Center for Practice Innovations at Columbia Psychiatry and New York State Research Foundation for Mental Hygiene.

PRIMARY FUNCTIONS:

• Establishing warm and collaborative relationships with community partners who might refer or provide ongoing support to young people and families
• Spreading awareness about OnTrackNY services through community education and outreach activities that emphasize OnTrackNY’s uniquely flexible and youth-friendly approach
• Collaborate with OnTrackNY team members to set new community-based goals focused on reducing inequities and barriers to accessing specialty care
• Identifying and creating new resources to support diverse young people and families seeking or receiving services at OnTrackNY
• Ensuring a welcoming and efficient process for young people and families seeking care in OnTrackNY
• Serving as primary contact person for referral sources and maintaining on-going communication with referral sources
• Conducting telephone and in-person eligibility screening of young people referred to OnTrackNY
• Referring participants ineligible for services to other appropriate community providers
• Establishing and maintaining database of referral sources with up-to-date contact information
• Attending weekly team meetings
• Participating in ongoing training and supervision
• Rotating on-call responsibilities with other team members
• Serves as the Primary Clinician for a subset of program participants. In this role as Primary Clinician, the Outreach and Recruitment Specialist will:
• Serve as the point person for the participant and their family;
• Provide services in the community (off-site) as needed based on participant and family preferences.
• Conduct needs assessments;
• Engage in safety planning and wellness management planning with participants;
• Connect participants and families with the services and supports they need;
• Provide behavioral interventions and take an especially active role in the family support domain.
• Conduct clinical assessment to determine the participant’s level of social functioning and substance use via a shared decision making (SDM) framework
• Provide Social Skills Training, Substance Abuse Treatment, Behavioral Activation, Coping Skills Training, and Psycho-education.
• Provide psychoeducation regarding first episode psychosis (FEP) and the goals of the OnTrackNY program;
• Provide supportive therapy;
• Develop and modify treatment using a shared decision-making process with clients
• Work with participants and family for transition from the team to other services and supports, and ensure stable links at discharge.

ADDITIONAL FUNCTIONAL / ORGANIZATIONAL SUPPORT:
• Additional duties as assigned

QUALIFICATIONS AND ATTRIBUTES:

• LMSW/LMHC/LMFT/LCSW or Limited Permit MHC/MFT
• Training and experience working with adolescents and/or young adults with serious mental illness.
• Experience in community-based mental health services

EDUCATION AND EXPERIENCE:

• Licensed Masters or Ph.D. in an appropriate field (social work, psychology, mental health counseling)
