
The LPN is responsible for ensuring a positive Skilled Nursing Facility experience for residents, their family members and visitors of the Skilled Nursing Facility. The LPN performs nursing procedures and treatments per the care plan and under the direct supervision of the registered nurse. Observes and documents resident conditions and communicates in a positive fashion with family, physicians and the entire care team. Must be confident in dealing with decision making, be able to manage time, pressure and deadlines in a positive fashion, and be able to respond appropriately.
Education Licensed Practical Nurse through the state of WI.
Certifications: BLS Certification required (can train on the job).
Shift Working 24 hours a week on the Day shift with rotating weekends and holidays.
Shift differentials offered as a percentage of base rate: 10% for PM shift and 17% for night shift
