SUMMARY The Contractor will provide quality care and technical skilled services, in the home setting, consistent with the clinical needs of the patient/client, and as the Contractor determines is necessary to promote the patient/client's health and client and as the wellbeing, in compliance with nursing standards, federal, state and local government regulations, and best clinical practices, within the parameters of the patient/client's plan of care. SCOPE OF SERVICES • Provides direct and individualized nursing care to assigned patients/clients, based upon nursing standards and the patient/client's plan of care established by the patient/client's clinical team Ensures quality and safe delivery of home health services to patients/clients and families/caregivers • Implements s plan of care formulated by patient/client's physician and/or registered nurse • Prepares and provides accurate and timely documentation consistent with patient/client's established plan of care • Assesses and provides patient/client and family/caregiver education and information pertinent to patient/client's diagnosis and plan of care • Assists in the maintenance of a safe and healthy environment • Administers services, including the use of equipment and supplies, safely and properly Participates in coordination of home health services with patient/client's clinical management team Assists patient/client in learning appropriate self-care activities Documents activities and findings and submits documentation to Company of such services performed to facilitate compliance with appropriate federal and state laws, rules and/or regulations with respect to billing, standards of care, and/or payment disbursements • Other tasks, which in the discretion of Contractor, contribute to the health and overall well-being of the patient/client within the Contractor's qualifications and in compliance with nursing standards and applicable laws, rules and regulations REGULATORY LIMITATIONS The Contractor performing Licensed Practice Nurse services will at all times adhere to the nursing standards and applicable laws, rules and regulations while performing such skilled services Requirements QUALIFICATIONS The requirements listed below are representative of the knowledge, skill, and/or ability required for the provision of Licensed Practice Nurse services. EDUCATION AND/OR EXPERIENCE Licensed Practice Nurse in state of practice in good standing;ng; Graduate of an accredited practice nurse program; At least one year of nursing experience; Community/home health experience preferred. LANGUAGE SKILLS Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form. Ability to communicate effectively with patient/client, family members, clinical management, and staff. REASONING ABILITY Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists Ability to apply common sense understanding to carry out instructions furnished in written, oral, or diagram form. Ability to deal with simple problems in the home setting. OTHER SKILLS AND ABILITIES Nursing skills, observation skills, and knowledge of home health care and government regulations. Caring attitude, tact, patience, and good personal hygiene; suitable disposition for relating to patient/client. PHYSICAL DEMANDS The services performed by the Contractor may require moderately heavy physical exertion on a regular and recurring basis such as: extensive driving, assisting patient/client in transfer activities (wheelchair, to bed, to tub, to commode) and providing substantial support to individuals in ambulation. While performing services as a Licensed Practical Nurse, the Contractor may be regularly required to use hands to finger, to handle or feel, and talk or hear. The Contractor may frequently be required to stand; walk; reach with hands and arms; and stoop, kneel, crouch, or crawl. The Contractor may occasionally be required to sit. The Contractor may occasionally be required to lift and/or move over 100 pounds. Specific vision abilities by a Licensed Practical Nurse may include close vision, color vision, peripheral vision, depth perception, and ability to adjust focus.
