Licensed Practical Nurse (LPN) – Hospice Care

Location:
Coverage areas include Burlington, Camden, Gloucester, Salem, Cumberland, Atlantic and Cape May Counties.

Pay Rate:
$36–$38 per hour

Shifts Available:
12-hour shifts 8:00-8:00 – Mostly Nights, some Days

We are seeking compassionate and dependable Licensed Practical Nurses (LPNs) to provide high-quality hospice care to patients in home and facility-based settings This role focuses on comfort, dignity, and emotional support for patients and their families during end-of-life care.

Responsibilities

• Provide direct nursing care to hospice patients in home or facility environments

• Monitor patient condition and report changes to the care team

• Administer medications and treatments as ordered

• Offer compassionate emotional support to patients and families

• Maintain accurate and timely documentation

• Collaborate with RNs, physicians, and hospice care teams

Qualifications

• Active New Jersey LPN license (required)

• Hospice or end-of-life care experience preferred (not required)

• Strong communication and clinical skills

• Compassionate, patient-centered approach

• Reliable transportation and ability to travel within assigned counties
