Pyramid Healthcare’s Charlotte ACTT Team is hiring a compassionate LPN or CMA to provide meaningful behavioral health and medical support directly in the community.

Our Assertive Community Treatment Team (ACTT) provides intensive, community-based services to adults living with severe and persistent mental illness. We meet clients where they are—helping improve stability, reduce barriers to care, prevent unnecessary hospitalizations, and support long-term recovery.

What You’ll Do

Nursing & Medical Support

• Coordinate care with providers, nurses, and healthcare partners
• Support medication compliance and medication management
• Monitor medication inventory, refills, and medical supplies
• Obtain and document vital signs
• Maintain accurate and timely clinical documentation
• Educate clients and staff regarding medications, wellness, and health needs

Community-Based Client Support

• Meet clients in their homes and community settings
• Transport clients to medical appointments and community resources
• Help clients access healthcare and other needed services
• Identify and help reduce barriers to treatment and continuity of care
• Work collaboratively with the ACTT team to support each client’s individualized treatment plan

Crisis Response & On-Call Rotation

• Participate in rotating crisis phone coverage
• Assist with crisis planning and hospital diversion efforts
• Provide support during behavioral health emergencies
• Assist with commitment procedures when clinically appropriate

Qualifications

✔ Active North Carolina LPN license OR Certified Medical Assistant (CMA) certification

✔ Valid driver’s license with ability to travel throughout the Charlotte service area

✔ Reliable transportation

✔ Comfortable working independently in community-based settings

Preferred

✔ Behavioral health experience

✔ Community health or ACTT experience

✔ Experience working with individuals with serious mental illness

✔ Strong communication, organization, and teamwork skills
