What You'll Do

• 
  Deliver skilled nursing visits throughout the Twin Cities Metro.
• 
  Perform wound care, medication management, and patient assessments.
• 
  Educate patients and caregivers on treatments and disease management.
• 
  Communicate closely with RN Case Managers, physicians, and the interdisciplinary team.
• 
  Complete accurate documentation using our electronic medical record.
• 
  Help patients remain independent and avoid unnecessary hospitalizations.

What We're Looking For

• 
  Active Minnesota
  Licensed Practical Nurse (LPN)
  license.
• 
  Strong communication and critical thinking skills.
• 
  Comfortable working independently in a field-based role.
• 
  Previous experience in skilled nursing, TCU, rehab, hospice, or home health is preferred.
• 
  Reliable transportation and valid driver's license.
