What You'll Do


  • Provide skilled nursing visits in patients' homes.

  • Perform wound care, medication administration, and disease management.

  • Reinforce education provided by the RN Case Manager.

  • Monitor patient conditions and communicate changes to the care team.

  • Collaborate with physicians, therapists, and fellow clinicians.

  • Complete accurate and timely documentation.


What We're Looking For


  • Active Minnesota
    Licensed Practical Nurse (LPN)

    license.

  • Reliable transportation with a valid driver's license and current insurance.

  • Ability to work independently and manage your daily schedule.

  • Previous nursing experience in skilled nursing, rehab, TCU, hospice, or home health is preferred.

  • Strong communication skills and a passion for patient-centered care.
